Myocardial deformation in patients with malignant diseases before treatment

Abstract

Assessment of myocardial strain using Speckle Tracking Echocardiography (STE), which is independent method of the scanning angle, allows to detect subclinical left ventricular (LV) dysfunction before the decrease of the LV ejection fraction. This concerns not only patients receiving anticancer therapy, but also patients with newly diagnosed cancer. Regardless of age, gender, body weight and other factors, a decrease in the global longitudinal strain (GLS) of both ventricles, as well as a decrease in the left atrium GLS in the reservoir and conduit phases along with an increase in pump function have been proven. Detection of this phenomenon in different tumor localizations suggests common mechanisms for the formation of cardiac dysfunction (hematological, genetic and cytogenetic factors), which are subject to further study. Modern literature emphasizes the role of strain in the primary assessment of cardiac risk in cancer patients.
